Profile:
Bob is a Strichting Tripod Foundation and Energy Institute accredited Tripod Beta Trainer and by invitation of the STF Board, an appointed Assessor. His association with Tripod Beta stretches back to 1990s where his technical knowledge of road tanker operations was key to understanding operational incidents. Bob joined Shell Southern Africa in 1986 as LPG Engineering Coordinator moving into Distribution Operational Training 18 months later. A function he managed until 1998 when he founded BOHIKA CC, a risk management and learning consultancy. Africa is known to be a hostile working environment and over the years Bob facilitated numerous Tripod Beta Incident Investigations and Analyses on behalf of Shell SnA and its many contractors. Most were transport related incidents, some with multiple fatalities. Other incidents include, armed robberies, falls from height, aviation refuelling, large product spills, tankage overfills, operational disturbances with and without injury.
Bob has had an association with the Shell Open University since 2004 delivering face to face and online learning events. He has worked with several other companies using Shell or similar HSE Management Systems. Bob has delivered Tripod Beta Workshops in Europe, Africa, middle and far eastern countries.

Lead Incident Management SME & Learning From Incident (LFI) Coordinator for Sub-Sahara Africa (UIG), ShellProfile:Uche has 24 years of Oil & Gas industry experience (as of 2013): 2 years with Intel Services (NICOTES) as Logistics Base Operations Supervisor for Oil & Gas companies and 22 years with Shell (1 year National Service & 21 years of employment).  Uche joined Shell in April 1992 in its Supply Chain department, Nigeria and worked as Fleet Control Supervisor, Land & Marine Transport Admin Supervisor, Procedures / Systems Analyst, Senior Field Operations Supervisor (including rig-move co-ordination and rig supplies – Heavy & Medium equipment).  Between 1997 and 2005 Uche transferred to the HSE department as Safety Advisor in the HEMP Team and HSE Management (Environmental Audit and ISO 14001). Team Leader – HEMP & Hearts & Minds, a role he was playing until his departure to Brunei Shell Petroleum in July 2005. At BSP Uche worked as Senior HSE Advisor in charge of Operations Services Departments (Land Transport, Air & Marine Logistics, Campaign, Engineering & Operations Capability/Maintenance) from 2005 to 2006. He became in charge of Technical Services Departments HSE deliveries (Geophysics, Studies / Developments, Geomatics, Exploration, Well Engineering & IT) from 2007 to 2010. Returned to Shell Nigeria (SPDC) in January 2011 to take up the Leadership of Incident Management SME and Learning from Incidents Coordination and embedment for Shell Companies in the Sub-Sahara African region (UIG).

John was accredited as one of the founding Tripod Assessors in 2000 and is an accredited Tripod Beta Trainer. He holds an Executive Certificate in Occupational Safety & Hygiene as well as Certificate IV in Training and Assessment. John designed the first Tripod Beta training course to be compliant with adult learning principles and much of his material forms the basis of the accredited training programs that exist today. John is a high performance coach facilitator with over 40 years experience in industry, including oil and gas and shipping and stevedoring both in Australia and overseas. He also has extensive experience in designing and facilitating health & safety initiatives and was instrumental in creating Woodside Energy’s Health, Safety & Environment competence model. He has developed a number of world-class behavioural based training programs to deliver those competences. Peter is an experienced incident and risk management analyst, management consultant, teacher/coach, and a certified information architect, specialising in: business process (re)design and information architecture; strategic, information & communications related consulting; risk assessment and -analysis & teaching analytical techniques.  His over 35 years of business experience include a broad variety of management and consulting positions at IBM (Europe), NCR Corp. Profile:Oscar studied Electronics (MBO) and Mathematics (HBO). After his studies he held various positions in the IT sector. During his role as ICT coordinator at AdviSafe Consultants (1996-1999) Oscar was closely involved in the development and automation of Tripod Delta questionnaire process and 'DeltaBase'.  Since 2006, Oscar has worked for AdviSafe Risk Management as consultant / trainer. Profile:Ed is accredited assessor, trainer and practitioner for Tripod Beta, experienced trainer and consultant for BowTie Risk Analysis, and Hearts and Minds programme. Ed Janssen studied chemical technology, inorganic materials at the Technical University Twente in the Netherlands and has over 25 years working experience with Shell. Prior to 2000, Ed was HSE policy advisor at Shell, responsible for development and coordination of HSE management systems. In that context also involved in the first developments of the Bow Tie risk analysis methodology, Tripod Beta accident analyses methodology, pro-active Tripod Delta and the Hearts and Minds programme. Since 2000, Ed has been an active independent consultant and trainer in the area of risk management systems, organisational culture and incident investigation acting in a network of specialists. Ken is an accredited Tripod Assessor and Tripod Beta Trainer. Ken first made use of the Tripod Beta principles in 1989 while working for Shell, and has been an accredited Tripod Beta Trainer since 2005. Ken started his career on large steam turbines with English Electric and worked on several power stations in the UK and the Middle East. He joined Shell International Exploration and Production in 1971 and worked for 28 years in many locations around the world. Ken worked for four years in Shell’s HSE department in Aberdeen, and was project leader for Shell’s two year, £10M major overhaul of the North Sea operations permit to work system following the Piper Alpha disaster. Ken then worked for five years in Shell’s international training centre in the Netherlands, before starting his own training and consultancy company EPEX in 1999. Since then Ken has delivered more than 100 Tripod Beta courses in more than 25 locations around the world.
