This technical publication is a user guide to accompany the workbook for the Failure mode and effects analysis tool for above-ground storage tanks storing petroleum, petroleum products or other fuels – Part 1: Failure mode analysis for primary containment (a.k.a. Part 1).
The workbook assesses the vulnerability of above-ground storage tanks (ASTs) (primary containment) to catastrophic tank failure (CTF) focusing on the factors that affect vulnerability to failure (causes) and the primary failure modes. It provides the results of a failure modes analysis using the failure modes and events analysis (FMEA) technique for CTF.
The AST failure rates determined in Part 1 may be used with the volumes of fluid released from failure or overtopping the secondary containment predicted in Failure mode and effects analysis tool for above-ground storage tanks storing petroleum, petroleum products or other fuels – Part 2: Effects analysis for secondary containment (a.k.a. Part 2) to inform a predictive CTF loss of containment (LoC) major accident hazard (MAH) risk assessment for ASTs and its secondary containment.
